In-Home Services

CAAC’s mission is to assist seniors who are at risk of losing their independence to remain at home and in their own communities for as long as possible. Through the coordination of a variety of services, these seniors are able to live with dignity in the comfort of their own homes and at much less expense than in a nursing or assisted living facility.

An array of in-home services is provided by CAAC through the Elderly and Disabled Medicaid Waiver Program. This program is designed to provide in-home and community-based services for low-income elderly and disabled persons, regardless of age, who might otherwise need nursing home care. The E & D Waiver Program is funded by the Alabama Medicaid Agency and administered through the Alabama Department of Senior Services. ADSS contracts with CAAC to provide the services.

Case Management is provided for every client. The Case Managers coordinate services, make regular home visits, develop and update the plan of care based upon an on-going needs assessment. They provide information and referrals for other community programs and assistance in applying for benefits and entitlements. A personalized care plan is developed for each client to meet the specific needs of the individual and may include any or all of the following services:

  • Homemaker Services include general household activities such as meal preparation, food shopping and routine household care provided by a trained homemaker when the individual responsible for these activities is absent or unable to manage the care and home.
  • Personal Care includes bathing, dressing, ambulation, assistance with eating, and supervising the self-administration of medications.
  • Respite Care (skilled or unskilled) is provided on a short-term basis, offering relief to the person normally providing care. A worker stays with the client in the client's home for a limited period of time, allowing the caregiver a break from the home.
  • Adult Day Health Care provides social and health care for individuals who need additional interaction or who can no longer remain safely at home alone, and also may allow the caregiver the freedom to continue to work or simply to have additional time for other needs and interests. Limited transportation is also provided for clients to attend the Day Care Center.
  • Companion Services provide non-medical care, supervision and socialization to a functionally impaired adult who lives alone. Companions may assist or supervise the individual with such tasks as laundry, meal preparation and other light housekeeping tasks. Companions may remind clients to bathe, care for their personal grooming and hygiene, and to take their medication.
  • Frozen Meals delivered to the home once per week. The client must have a microwave or traditional oven for the heating of the meals and freezer space to accommodate a week's supply of meals.
